The LPN/LVN Nursing Coordinator assists the Registered Nurse with oversight of routine functions of an assigned nursing unit to include functioning as a Charge Nurse and ensuring the delivery of nursing care to residents, as assigned and supervised by an RN, as prescribed by the physician and in accordance with standards of nursing practice and regulations.
ESSENTIAL JOB DUTIES
Assist in scheduling job assignments and developing nursing unit priorities as assigned and supervised by an RN. Monitor work assignments, provide feedback, and assist an RN in reviewing the day-to-day delivery of resident care. Advise the R.N. of care delivery and staff performance concerns as needed
Participate, under the direction of an RN who establishes and directs nursing care, in the interviewing and hiring of nursing assistant applicants. Under the direction of an RN, review certain non-nursing practice performance of nursing assistant employees, to include the effective recommendation of suspension and termination
Prepare and review medical records and nurses' notes under the direction of an RN and participate in the on-going observations of residents to determine their physical and emotional status. Monitor resident activity and provide nursing care as directed by an RN, according to the physician's orders, care plans, established standards and facility policies. Coordinate and assist or prepare for new admissions, transfers, and discharge of residents as directed by an RN. Transcribe physician's telephone orders and review with an R.N. Communicate with residents, family members, other interdisciplinary team members, and management
Prepare and administer medications under the direction of an RN and as ordered by the physician in accordance with nursing standards and facility policies. Sign, dates and perform all charting and recordkeeping in accordance with established policies and procedures
Understands, complies with, and promotes all rules regarding residents' rights
Assist RN as directed and participate in implementing a written care plan for individual residents that addresses the needs of the residents
Attend and participate in training, assist an RN in implementing orientation programs and job skills training within the scope of practice for the licensed practical nurse; maintain professional competence
Monitor environment and care practices of nursing personnel as directed by an RN to ensure compliance with established safety, fire and disaster, infection control and all other departmental policies and procedures are followed; and to ensure cleanliness and safety of work and treatment areas
CODE OF CONDUCT
- Must adhere to the Company's Code of Conduct and Business Ethics policy including documentation and reporting responsibilities
Must be a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) in good standing and currently licensed by the State
Completion of secondary education or otherwise show ability to read and write in a legible and understandable manner
Related work experience is preferred
Must be capable of maintaining regular attendance

Must be able to maintain confidentiality regarding patient and company proprietary information
Must be able to relate positively and favorably to residents, families and other employees at all levels
PHYSICAL AND SENSORY REQUIREMENTS
- Mobility, reaching, bending, lifting, grasping, fingering, fine hand coordination, ability to talk and hear; ability to read and write; ability to communicate with personnel, ability to remain calm under stress. Ability to adequately lift, support and bodily manipulate the weight of a patient.
