The Social Worker Program Coordinator functions as the Grant and Per-Diem (GPD) Liaison for the Chalmers P. Wylie Veterans Outpatient Clinic Health Care for Homeless Veterans (HCHV) GPD program.

Responsibilities

Work Schedule: Monday through Friday 8:00 a.m. to 4:30 p.m. Fi*nancial Disclosure Report:* Not required The primary purpose of the position is monitoring and coordinating services provided by community-based programs funded under the VA's Homeless Providers GPD program. The liaison is the key point of contact between the programs in the field and the VA Central Office. Position requires significant travel within the community. Major duties include:

  • Provides supportive services and case management to veterans residing at GPD facilities;

  • Monitors the clinical care and assesses the compliance of the programs receiving per-diem payments, as described in the recipient's original grant application and intervenes when appropriate;

  • Provides oversight of the GPD funded programs as defined by VHA policy;

  • Coordinates and participates in program site inspections utilizing GPD central office guidance and policy;

  • Coordinates care and acts as an advocate on behalf of veterans to ensure comprehensive service delivery and provides linkages for veterans to health care services and community agencies;

  • Assists the Coordinator, Community Outreach Division, with ensuring the HCHV program meets CARF, SOARS and Joint Commission requirements.

Qualifications

**Basic Requirements

  • **US Citizen;

  • Master's Degree in Social Work from a school of social work fully accredited by the Council on Social Work Education (CSWE); -Licensed by a state to independently practice social work at the master's degree level (advanced practice level)**;

  • **Completion of a minimum of one year of post-MSW degree experience at the GS-11 full performance level. Experience must demonstrate possession of advanced practice skills and judgment. Advanced Practice Level is defined as the ability to provide independent and expert clinical psychosocial and case management services in a specialized area of practice to veterans who tend to have serious and severe crises, may lack any familial and community support, may be poor self-monitors, may frequently fail to comply with instructions and treatment, or may have major deficits in coping skills and require continuing professional psychological support.
